I wanted an assessment portfolio that aligned perfectly with our report card so that when I assess the kids and fill out the report cards, everything would be in one spot. I also included spaces for cutting samples, self-portraits, monthly writing samples, and photos. AND everything is Common Core aligned with the standard and a link to each standard under each (and every) skill. It was a lot of work, but I think I'll really like it.
